Dear Mr. Baregi:
The City is in receipt of your additional application materials submitted on February 24,
2016 for the redevelopment of the Holiday Stationstore at 14773 60th Street North in
Oak Park Heights. The application consists of a Conditional Use Permit for Fuel Station
and Convenience Store, Conditional Use Permit for Signage, Preliminary and Final Plat,
and Design Guidelines/Site Plan Review. With this letter we are notifying you that the
application is not complete. The City will deem it complete with receipt of the following
items by March 1, 2016 at 2:00 PM:
1. Provide the application and consent information as requested by the City
Attorney:
Signature on the consent is still not phrased in the corporate ownership. Owner
is McCullough & Sons Inc. The application and consent appear to be listed and
signed in the name of individuals. Corporation must sign and corporate titles and
authorization must be displayed. The Consent should be revised to
add:...." is authorized by McCullough & Sons Inc to apply for and obtain
any and all development and land use permits of the City of Oak Park Heights
relative to the lands identified within the application and McCullough & Sons, Inc.
agrees to be bound by the terms and conditions of any permits and approvals
issued thereby.
2. The diagram "Holiday Oak Park Heights Addition" should be placed in a
Preliminary Plat format consistent with the requirements of Article 402 of the City
Subdivision Ordinance. The Plat should be clarified as to if the right of way will
be a separate lot and combined with what is now identified as Lot 2.

3. As to park dedication, the City will accept the value per square foot indicated at
the meeting as a placeholder, but will review the issue further and request
additional verification if necessary.
