AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to:(1)the conditions of this permit;(2)the
approval plans and specifications;(3)the applicable city
approvals,Ordinances,and Codes;and,(4)the State Building
Code.This permit is for only the work described,and does
not grant permission for additional or related work which
requires separate permits. This permit will expire and become
•
null and void if work is not started within 180 days,or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

Grading and Utility Permit—Supplemental Conditions
The City does conditionally issue the attached Grading and Utility Permit(s) allowing the placement of an
infiltration basin at the District's property conditioned upon the items noted below. By acceptance of this
permit(acknowledged by signing below)and the subsequent installation of related items under this permit,
the Stillwater Area School District (the Developer) shall have acknowledged and accepted each of these
conditions:
1) Prior to the start of any grading or utility installation operations,the Valley Branch Watershed shall provide
written summary documentation and reports to the City Administrator indicating their unambiguous and unilateral
approval of the proposed infiltration basin and detailed rationale as to why they are an appropriate MS4 agency to
make this finding and request the City approve the installation. The required documentation shall include the"higher
engineering review,sufficient to provide a functioning treatment system and prevent adverse impacts to groundwater"
as indicated in the MPCA's General Permit for Authorization to Discharge Storm water for allowing infiltration within a
Drinking Water Supply Management Area(DWSMA).
2) If at any time,the MPCA or other regulatory agency determines that an infiltration basin may not be approved
by the City as outlined under the above paragraph and/or determines that a infiltration basin is not permitted within the
City's DWSMA,the School District(applicant)shall be responsible for any and all remedial actions,fines, penalties or
other any necessary actions to satisfy or correct any condition or action complained of by such agency, including but
not limited to,the removal of the infiltration basin.It is understood that the infiltration basins are not the property of the
City and that all future maintenance shall be performed by the District.
3) All final utility and grading plans shall be subject to the approval of the City Engineer and such approved plan
shall be followed. The last reviewed dated plans were dated 1/19/16—Addendum 2 as recently reviewed by the City
Engineer with the latest comments being issued on 3/10/16 as attached.No grading or utility alterations shall be made
without the written approval of the City Engineer which shall not be unreasonably conditioned or delayed.
4) The School District shall develop and put in place appropriate response measures and procedures to ensure
that any spills or other adverse releases that may occur into any such infiltration pond or basin to be constructed under
this Permit shall be immediately responded to,managed and reported to the City,the DNR or MPCA Duty Officer and
other required agencies.

Reference: Stillwater High School Addition—244 Plan Review
Dear Eric,
We have reviewed the re-submitted plans for the Stillwater High School Addition and Pony Activity
Center received March 3,2016. Following are our comments and/or recommendations:
Sheet 200.CG
1. At the west entrance to the school off the County Road,the grades at either side of the driveway
are both indicated to be 937.40. Provide detail that shows how the drainage will work in this
area.
2. The proposed right turn lane on 58th Street needs modification. It appears that the cross slope
for the turn lane is currently designed to flow away from the curb toward the point where the
new turn lane pavement will meet the existing roadway pavement. This will create an
"inverted"section between the new turn lane and existing pavement. The cross slope of the
existing roadway needs to be carried through all the way to the curb of the new turn lane so that
the drainage runs along the curb,not along the road at a seam between the turn lane and the
existing roadway. Incorporate the appropriate additions to the storm sewer system and
include the corresponding City detail plates. Submit the supporting drainage calculations.

1. The City's standard curb and gutter section is B618;currently B612 is shown on the plans for
the proposed turn lane on 58th Street. Please revise for curb along 58th Street and include the
City's detail plate STR-1.
2. In the new parking lot on the northwest part of the site,top of curb and gutter line grades are
shown in the pavement areas(no curb).

1. Utility note#11:Testing needs to include pressure testing,bacteria testing,conductivity
testing and any other testing required by the City's Utility permit.The City will receive copies
of all testing results.

2. Add a testing note for sanitary sewer. Testing of sanitary sewer needs to include air testing,
mandrel testing and televising. The City will receive copies of the testing results and the
televising videos
3. At the southeast addition to the building,show the hydrant lead to the salvaged hydrant shown.
4. Show the storm sewer design for the proposed turn lane on 58th Street.

1. South of the existing building,the plans indicate lowering the water main by the salvaged
hydrant. Please clarify this lowering in comparison to the vertical location of the sanitary
sewer.
2. Include testing notes as indicated for sheet 200.CU

1. Include City Detail Plate STR-1 for B618 curb.

1. Replace Detail 4,RipRap At Outlets,with City Detail Plate STO-13.
2. Include City Detail Plates WAT-3,Gate Valve and Box Installation;and WAT-6,Concrete

Storm Water Management
1. The issue with allowing infiltration in the City's DWSMA is resolved as documented by the
letter from the City of Oak Park Heights to the Minnesota Pollution Control Agency dated
February 23,2016. As stated previously,this facility will be privately owned and maintained
by the School District.
2. Provide the City with a copy of the permit approval letter for the project from the Valley
Branch Watershed District.

General Comments
1. The School District will provide the City with complete detailed as=builts of the utilities on
site.
2. The City reserves the right to inspect the installation of utilities,any work in the City's rights-
of-ways and easements,and any work that impacts a City system.
