LISPS Stillwater/Oak Park Hei Carrier Annex — Oak Park Hei Minnesota VBWD Permit #00-16 Dear Mr. Gustafson: Enclosed is the Valle Branch permit for y our project. Please note the followin conditions imposed b the Mana which are also listed on the back of the permit. 1. Erosion control blankets should be installed on all disturbed slopes three feet horizontal to one foot vertical or steeper. 2. Disturbed areas should be seeded and mulched within 14 da of final g radin g . 3. Riprap sizin calculations should be submitted that show the riprap is lar enou to handle dischar up to the 100- critical storm event. 4. The cla liner at the stormwater pond shall onl extend three feet up from the pond bottom ( to Elevation 937). 5. The n*prap from the curb cuts should be extended down to at least Elevation 937 (the top of the cla liner) . 6. The stormwater basin should be ve with deep-rooted, native ve that can withstand periods of inundation. 7. The VB" En and/or Inspector shall be notified at least three da prior to commencement of work to schedule an inspection of the project's erosion controls. The erosion controls must be in place and properl 'installed before g radin g ma commence. 8. Draina easements shall be g ranted to the Cit of Oak Park Hei which cover land adjacent to the pondin area up to its 100- flood elevation and which cover all ditches and storm sewers. Access easements to these drainage easements shall also be g ranted to Oak Park Hei The cit of Oak Park Hei shall be responsible for all maintenance within draina easements. The 100- flood elevation for the stormwater pond on the site is Elevation 941.7. 9. The minimum buildin elevations for the lot shall be recorded in a Declaration of Covenants and Restrictions or on the final plat. The minimum buildin elevations for the lot is 943.7. 10. The deed shall state existence of draina easements and wetlands where applicable i i. This permit is not transferable. 12.
