AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1)the conditions of this permit;(2)the
approval plans and specifications;(3)the applicable city
approvals,Ordinances,and Codes;and,(4)the State Building
Code.This permit is for only the work described,and does
not grant permission for additional or related work which
requires separate permits.This permit will expire and become
null and void if work is not started within 180 days,or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.
SEPARATE PERMITS REQUIRED FOR WORK OTHER THAN DESCRIBED ABOVE.

Project Sunnyside Condo's-Repairs to two roof areas.
Repair scope for area(2):
• Remove deck and railings from repair area.
• Remove siding as needed to access the wall flashing.
• Remove flashing on one side of skid.
• Remove all gravel from repair area.
• Remove roofing from repair area.
• Remove all wet insulation from repair area.
• Install new insulation to repair area.
• Install 3 plies of roofing felts in hot asphalt.
• Install new base flashing to building wall,skid,and outside canted
edge.
• Install two coats of 838 Supreme coating to the new 3ply roofing
felts.
• Install deck back in same location.
• Replace deck screws as needed due to fastener deterioration.
• Install siding as needed.
• Remove all debris from job site.
Area 2 for the sum of:$14,975.00
Notes:
• To follow OSHA fall safety,we will be hoisting and using fall safety
carts during the project.
• Building permit included.
• Dumpster and hoisting included.

• Remove metal coping as needed.
• Remove base flashing in repair area.
• Remove roofing and wet insulation.
• Install new insulation and taper edge along wall area.
• Install 3 plies of roofing felts in hot asphalt.
• Install gravel over repair area.
Area 1 For the sum of:$3,995.00
Notes:
• The wood blocking repairs will need to be completed prior to the roof
repair.
• Scope of repair subject to change depending on contractors need for
structural repairs.
