We have received the "letter of consent" from Bremer Bank for Net Lease to demo existing funeral home. 1 have faxed a
copy of the letter to you Mark.
Jim and Jay
I've prepared the permit, incorporate the legal notes from Mark Vierling into the note section and have advised Mr. Totzke
that he may pick the permit up on Monday. The fee has been based on the valuation of $40,000.
I would like both of you to let me know of any special inspections or notes you would like added a /s /a /p so that I may add
them. As of this moment, the only inspection noted in the final.

Subject: Walgreens Demoliton Permit - Information Update
I've just spoke with Mark Vierling as to the supplemental information needed by Walgreen's for demolition permit approval.
Mr. Vierling has stated that the permit may be issued upon receipt of a Letter of Consent from Bremer Bank for the
existing building to be demolished.
I have contacted both Mr. Tom Totzke's office number and cell number and have left detailed voice mails that the letter of
consent is needed prior to permit. I have stated that he may have the bank fax this letter over in order for me to get the
permit processed, but that the original would need to be received before I would issue the permit.
If he happens to call or the fax comes through, please let me know. It is my understanding that they plan to demo this
Monday.

1 am not familiar with Captial however 1 have been communicating with Tom Totzke of Net Lease
Development and BTS Construction LLC regarding permt for demoltion of the Bradshaw Funeral Home as part of the
Walgreen's Construction project.
I've spoken ith Mr. Totzke regularly, including this morning and have let him know the status as of the matter as | have
been Iast advised | have reiterated my promise to him that as soon as | receive word back on his recent application
supplemental material, i will call him.

Conditions for Use of Hydrant:
1. Hydrants will only be used with external valve and backflow prevention device approved for the conditions of
use as specified by Minnesota Health Department Plumbing Codes.
2. The hydrant main valve will only be operated fully open and the water flow regulated by the external valve. The
external valve will be closed prior to opening the hydrant valve and closed prior to closing the hydrant valve.
External valve should be removed from the hydrant in a closed.
3. When closing a fire hydrant the valve is held closed by the watermain line pressure. When properly closed the
valve nut should have free play of 1/8 to 1/2 of a turn. Excessive tightening of the valve will damage the valve
and result in this permit being revoked and damages for repair of the hydrant assessed to the permit holder.
4. If the external valve is to be locked when not in use or under the direct supervision of the permit holder.
Hydrant wrenches or external valves left unsupervised and unsecured will be removed from the hydrant and the
permit revoked.
5. Tank filling will utilize an air gap devices as specified by the Minnesota Department of Health Plumbing Codes.
6. Report any hydrant problems to the public Works Department.
7. Permit holders measuring use by numbers of loads will report the number and size of loads to Public Works on
a monthly basis.
8. This permit is valid for the duration and hydrant location specified.

Thank you for faxing the information for the demoliton permit over so quickly. \ forwarded the same to our City Attorney to
confirm that the documentation is what was desired. At this time, 1 have been advised that an inquiry has revealed that
the fee owner does not match country records and that the City Attorney's office is looking into the matter. have been
directed to hold the permit until I received word from their office that the permit may be issued. We will contact you
immediately upon receiving that direction. Please be reminded that no demolition work should occur until such time the
permit has been inssued.
Secondly, the fee for the pemn�does include a plan rev�w.Aeoreou|tj�eadd�ona| amount of$353.11 should boadded
to the �g`re provided you with yesterday, when you stopped by. The total fee for the demolition permit will be $91 636.
I apologize foran inconvenience created and will be in tcuch to keep you posted as 1 learn anything relative to getting this
