Oak Park Heights, MN 55082 Ref: Occupancy at 5620 Memorial Ave. N Dear Mr. Zintl, On April 15, 1995, Valley Inspection Services issued two permits for the above address. Building Permit number 155 for an addition and Building Permit number 156 for a remodeling project. In reviewing the documents that I have received from Baytown Township and Washington County, I have no record that an Amendment to your Conditional Use Permit was applied for, or obtain. I must inform you that prior to the issuing of a Certificate of Occupancy the City of Oak Park Heights must have a copy of this Amendment as approved by Baytown Township. If the Township did not issue an Amendment to your Conditional Use Permit you must apply for the same with the City of Oak Park Heights. In my review of the plans and the permit issued by Valley Inspection Services I found the following conditions to exist: 1 The building type of construction was noted as being 11-N. we discussed this issue and agreed that the type construction, due to the wood floor system should have been listed as I1I -N. 2) The Occupancy was listed as E-1 with the building's total square footage of 9,420 feet. With these conditions the Building Code requires a sprinkling system to be installed. Plans must be submitted for approval and the system installed prior to the issuing of a Certificate of Occupancy. At this time the water supply to the building is by a well which would require that a holding tank be installed. I am aware that this can become cost prohibitive and suggest that an alternative solution might be granted by the City. If you have any questions regarding the above issues please contact me.
