Oak Park Heights, MN 55082 NOTICE OF COMPLETED PLAN REVIEW Dear Mr. Lindquist: The Minnesota Department of Agriculture (MDA) has completed the review of food equipment and remodeling plans for the Kowalski's Market located at 5801 Neal Ave. N., Oak Park Heights, Minnesota. This review was based on the plans submitted with the application, including revisions agreed upon during discussions that may have occurred throughout the plan review process. Please see the summary of the review below which includes any revisions or deficiencies that must be addressed prior to the pre-operational inspection. Any additional revisions must be submitted for review and approval which may delay final approval. Remodeling of the food establishment may now begin. When remodeling is complete and all noted considerations and deficiencies have been addressed, please contact the area inspector to arrange a pre-operational inspection: • Adam Lewis at (651)503-6408 or adam.lewis@state.mn.us. Be sure to give the inspector two weeks' notice before the opening date, as required by Minnesota Rule 4626.1750. Note: The newly remodeled area cannot be stocked with food until approval is granted by the inspector at the time of the pre-operational inspection. Project Description: This review was for the remodel of 9 square feet within and existing licensed facility in order to install a freestanding, customer self-service kegerator kombucha beverage station. Menu: Adding a kombucha beverage. Room Finish Schedule: No changes to the existing retail floor finishes where the kegerator will be installed. Deficiencies: No deficiencies. Mr. Lindquist August 16, 2019 Page 2 of 2 Other Considerations: 1. Kombucha unit is located near the Dairy Department. 2. A ready-to-tap keg of kombucha will be provided by a separately licensed vendor. 3. The kegerator will be used for kombucha only. 4. There is an automatic condensate evaporator, so no drain connection required. Evaporator condensate is plumbed to a condensate pan located in the compressor housing. 5. Follow the kegerator manufacturer's instructions for the proper cleaning of the tap dispenser and lines. Special cleaning procedures are required. 6. At the kegerator station, customer are on their own to fill growlers brought from home, or fill growlers purchased at the kombucha station. The following items remain the responsibility of the establishment and are required as part of the retail food establishment licensing process.
