AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1)the conditions of this permit;(2)the
approval plans and specifications;(3)the applicable city
approvals,Ordinances,and Codes;and,(4)the State Building
Code. This permit is for only the work described, and does
not grant permission for additional or related work which
requires separate permits. This permit will expire and become
null and void if work is not started within 180 days,or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

This review is limited to the provisions of the Minnesota Plumbing Code,Minnesota Rules,Chapter 4714 and
assumes the data on which the design is based are correct. Approval is contingent upon meeting the
requirement(s) listed below. A copy of the approved plans and this report must be retained at the project
location.
INSPECTIONS: All plumbing installations must be tested and inspected in accordance with the requirements of
the Minnesota Plumbing Code. No plumbing work may be covered prior to inspection. The contractor/installer
must obtain all required inspection permits from the Oak Park Heights Building Official,Julie Hultman.
REQUIREMENT(S):
1. All plumbing shall be installed in accordance with the 2015 Minnesota Plumbing Code, Chapter 4714.
2. There are several fixtures that are being replaced. You must verify the individual waste,vent, and water
supply systems are installed in accordance with the current Minnesota Plumbing Code.
3. Each plumbing fixture trap, except as otherwise provided in the code, shall be protected against siphonage
and backpressure,and air circulation shall be ensured throughout all parts of the drainage system by means
of vent pipes installed in accordance with the requirements of Chapter 9 and as otherwise required by code
(see Section 901.1).
4. Capping off water distribution piping should occur as close as possible to the distributing main to prevent
the creation of dead ends.
5. Where damage may be caused by leaking,water heaters must have water-tight corrosion-resistant drainage
pans(see Sections 507.4 and 507.5). A 3/4-inch minimum drain must extend from the pan to an approved
location. The water heater relief valve must extend independently to a safe place of disposal and may not
discharge to the pan.
6. PVC solvent weld joints must use ASTM F656 purple primer and ASTM D2564 cement per Section
705.7.2.
7. The plumbing system shall be tested in accordance with Sections 609.4 and 712.0.

NOTE(S):
1. The scope of this project consists of remodeling an existing building. Installation includes four water
heaters,three pressure vacuum breakers for the chemical dispensers,a drinking fountain,three mop sinks,a
hand sink, a break room sink,and water closets.
2. The building is served by existing municipal sewer and water services.
3. Plans and specifications must be approved by the Minnesota Department of Agriculture,Dairy&Food
Inspection Division. Please call 651/201-6027 regarding plan review and licensing requirements.
Authorization for installation may be withdrawn not undertaken within one year. Additional recommendations or
requirements may be made if changed conditions or additional information make improvements necessary.
