AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1)the conditions of this permit;(2)the
approval plans and specifications;(3)the applicable city
approvals,Ordinances, and Codes;and,(4)the State Building
Code. This permit is for only the work described,and does
not grant permission for additional or related work which
requires separate permits. This permit will expire and become
null and void if work is not started within 180 days,or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

This review is limited to the provisions of the Minnesota Plumbing Code,Minnesota Rules,Chapter 4714 and
assumes the data on which the design is based are correct. Approval is contingent upon meeting the requirements
listed below. A copy of the approved plans and this report must be retained at the project location.
INSPECTIONS: This project will be inspected by the local municipality. The contractor/installer must obtain
all required inspection permits from the Oak Park Heights Building Official, Julie Hultman. All plumbing
installations must be tested and inspected in accordance with the requirements of the Minnesota Plumbing Code.
No plumbing work may be covered prior to inspection.
REQUIREMENT(S):
1. All plumbing shall be installed in accordance with Chapter 4714. All pipe, fittings,traps,fixtures,
materials, and devices shall be listed or labeled by a third-party listing agency and comply with the
applicable standards referenced in the code(see Sections 301.1 and 1401.1).
2. The new water distribution branch must connect to an existing branch of at least I-inch in size an in no case
smaller than will meet the requirements of Section 610.
3. Double wyes may not be used for drainage fittings in the horizontal position(see Section 310.5). Proper
pipe slope cannot be maintained on both of the offset branches.
4. Trap primers must have an ASSE 1013 reduced pressure principle backflow preventer; an ASSE 1020
pressure vacuum breaker; an ASSE 1056 spill-proof vacuum breaker; or an ASSE 1001 atmospheric
vacuum breaker with no shutoff valve downstream(see Table 603.2). Trap primers with integral backflow
prevention are permitted only when equipped with one of the above-listed backflow preventers.
5. A hose thread water outlet without listed integral backflow prevention must be provided with an ASSE 1052
non-removable hose bibb-type backflow preventer; an ASSE 1011 non-removable hose bibb type vacuum
breaker; or an ASSE 1001 atmospheric vacuum breaker on the discharge side of the last control valve at
least 6 inches above the highest point of usage(see Sections 603.5.7,603.4.6, and 301.1.2). This shall
include the laundry tub faucet.
6. The water heater relief valve may not discharge to the pan.
7. The installation of reduced pressure principle assemblies,double check valve assemblies,pressure vacuum
breakers,or spill-proof vacuum breakers is permitted only when a testing and inspection program acceptable
to the administrative authority is provided (see Section 603.5.23). The administrative authority and water
purveyor must be notified prior to installation. Devices must be tested upon initial installation and not less

than annually,and records must be kept. Installations must be at least 12 inches and not more than 5 feet
above the finished floor or ground level unless a permanent platform for access is provided. A backflow

8. Public-use lavatories must have ASSE 1070-2004 mixing valves limiting the hot water temperature to 110
degrees Fahrenheit or less(see Section 421.2).
9. The water closets must have open-front seats or automatic seat cover dispensers(see Section 411).
10. Unions shall be installed not more than 12 inches from water heating or regulating equipment,water
conditioning tanks,and similar equipment requiring service by removal or replacement(see Section 609.5).
1 I. All sanitary drainage pipe must be sloped at 1/4-inch per foot minimum(see Section 708.1). Where site
conditions preclude this slope,pipes 4-inch and larger may be sloped at '/-inch per foot where approved by
the administrative authority.
12. Each horizontal drain branch on the lowest floor level of the building, including floor drain branches,shall
be provided with a cleanout at its upper terminal. Cleanouts are not required on branches less than five feet,
unless sinks or urinals are served(see Section 707.4).
13. A cleanout is necessary where new waste and vent piping connects with existing for testing.
14. Pipe materials used in the plumbing system must meet the following:
a. Water distribution piping must comply with applicable standards in Table 604.1:
i. Copper water distribution pipe must meet ASTM B42,B75,B88,B251,B302,or B447 (see
Table 604.1). Solder and flux shall contain less than 0.2 percent lead. Joints must use ASTM
B813 non-corrosive non-toxic paste-type flux(see Section 605.3.4).
ii. Cross-link polyethylene(PEX)water distribution systems must meet Section 605.10:
a. The tubing must comply with ASTM F876,ASTM F877,or CSA B137.5.
b. ASTM F876 tubing must be marked with the standard of the fittings to be used.
c. The fittings must be marked with ASSE 1061,ASTM F877,ASTM F1807,ASTM
F1960,ASTM F1961,ASTM F2080,ASTM F2159,ASTM F2735,or CSA B137.5.
iii. CPVC water distribution systems must meet Section 605.4:
a. Pipes must meet ASTM Standards D2846, F441,or F442.
b. Solvent cement must comply with ASTM F493. Solvent cement requiring the use of
primer must be orange. Primer must meet ASTM F656 and be colored. One-step yellow
or red solvent cement is permitted for'/2-inch through 2-inch ASTM D2846 pipes only.
c. Installation Standard 20 must be followed,including provisions for expansion and
contraction.
b. Drain,waste,and vent piping must comply with Table 701.1. If plastic pipe is used:
i. ABS must meet ASTM D1527, D2661, F628,or F1488 per Installation Standard 5.
ii. PVC must meet ASTM D1785, D2665, F891,or F1488 per Installation Standard 9.
iii. Above-ground horizontal pipes must be supported at each horizontal branch connection.
Restraint fittings or a minimum 24-inch offset using 45-degree fittings should be provided every
30 feet. Vertical runs exceeding 30 feet may have approved expansion joints installed per the
manufacturer's instructions.
iv. PVC solvent weld joints must use ASTM F656 purple primer and ASTM D2564 solvent cement.
Future submittals must include material specifications.

15. The plumbing system shall be tested in accordance with Sections 609.4 and 712.0.
16. The water distribution system shall be disinfected in accordance with Section 609.9.
NOTE(S):
1. The scope of this project consists of the remodelling of an existing building. The plumbing installation
includes a water closet,a lavatory,a laundry tub, four counter sinks,and a drinking fountain.
2. This facility will be served by existing municipal water and sewer services.
3. The current Minnesota Plumbing Code, Chapter 4714, and related information can be found at:

Authorization may be withdrawn if installation does not begin within one year. Additional requirements may
result from changed conditions or additional information.
