Dear Mr. Sexton:
The City is in receipt of your application materials submitted on April 15, 2021 for the
addition of a Pet IQ within the Walmart at 5815 Norell Avenue North in Oak Park
Heights. The application consists of requests for Conditional Use Permits for signage
and Design Guidelines Review for the addition of the exterior door. With this letter we
are notifying you that the application is not complete. The City will deem it complete
with receipt of the following items by April 28, 2021:
1. Provide mailing labels for properties within 500 feet of the exterior boundaries of
the property. The labels are available from Washington County.
2. Provide a site plan and floor plan of the building indicating where the Pet IQ will
be located within the structure and where on the site the entrance will be in
relation to the parking lot.
3. Include in the narrative an explanation of how Pet IQ will communicate to the
customer where to find the entrance to the Pet IQ and not try to access through
the main entrance. Plan A103 indicates a door into the interior of the Walmart.
Will customers be able to access Pet IQ through that door?
4. Explain how the entrance will be disability accessible.
5. The plans indicate a number of small signs on the exterior. In that Walmart is at
its limit on numbers of signs, we request that you revise your signage to one or
two exterior signs.
6. Will there be any changes to the exterior lighting to facilitate access to your
entrance? If so, provide a lighting plan.

The Planning Commission would consider the application at a meeting scheduled for
May 13, 2021 at 6:00 PM. City Staff will contact you regarding the agenda and Zoom
access prior to the meeting.
Pursuant to Minnesota Statutes 15.99, Sub. 3(f), the City of Oak Park Heights is
herewith extending the period of time for agency/city review of the application for an
additional 60-day period (for a total of 120 days from the date of a complete application)
inasmuch as the City has determined that additional research and study by the City staff
relative to the complexity of the project as proposed will be required.
