The following are comments and recommendations from the March 6, 2020 Planning Report for the
proposed Twin Cities Orthopedics at Memorial Avenue North.TCO Civil Plans prepared by Westwood
have been revised and are dated 6/11/2020 and address the following comments. +5esponses to the
comments are provided in red italic
City staff recommendations:
1. The drainage and utility easement vacation and establishment of the new easement is subject to
review and approval of the City Engineer and the City Attorney. 4cknowledged—exhibits for
easement vacation and establishment of a new easement were previously submitted to the City
for review.
2. The site plan shall be subject to review and approval of the Fire Marshal. Acknowledged. The site
layout and truck turning movements were reviewed and approved by Allen Eisinger, Fire Chief via
email correspondence dated 2/26/2020.
3. The applicant shall provide a snow removal pain or indicate if the snow will be hauled off site.
Snow storage areas are shown on the Site Plan (see sheet C200)
4. The landscape plan shall be subject to the reveiw and approval of the City Arborist.
Acknowledged. Response to the City Arborist comments are addressed below.
5. The Applicant has been encouraged to show some of the parking as proof of parking spaces,
possibly the row of parking at the south side of the building,The Planning Commission should
comment. Acknowledged. This item was discussed at the City meetings. TCO is proposing that all
parking to be constructed as they anticipate the need for all stall for daily operation.
6. All grading, drainage and erosion control, and utility plans are subject to review and approval of
the City Engineer. Stormwater plans are also subject to review and approval of the Browns Creek

Watershed District.Acknowledged. Response to the City Engineer comments are addressed
below. The plan will be forwarded to Browns Creek Watershed District for review.
7. All light fixtures shall be full cut off, compliant with the height requirements,and be consistent
with Section 401.15.6.7 of the Zoning Ordinance as it relates to lighting requirements. Outdoor
lighting for the parking area shall be turned off one hour after closing,except for approved
security lighting.Acknowledged. This item was discussed at the City meetings.
8. The proposed LED strip lighting shall not be allowed. Acknowledged. This item was discussed at
the City meetings.
9. All mechanical equipment on the roof or on the site shall be fully screened in accordance with the
provisions of the Zoning Ordinance.Acknowledged. See Architectural plan prepared by others.
10. The second wall sign reading Orthopedic Urgent Care on the north elevation shall be removed if
that service is discontinued at this location. AcKnowieaged. This item was discussed at the City
meetings
11. The Applicant shall provide sign detail including the square footage of each wall sign and the
freestanding sign.Acknowledged. See signage plans prepared by others
12. All signs shall be designed to comply with the Zoning ordinance requirements for fixtures cutoff
and illumination.Acknowledged. See signage plans prepared by others.
13. The Planning Commission shall comment on the building design and building materials.
Acknowledged. This item was discussed at the City meetings.
14. A bike rack shall be installed near the front building entrance. Bike rack is shown on the Site Plan
(see sheet C70n!
Plan review comments prepared by Stantec Consulting Services, Inc.
Sheet C100-200—Existing Conditions and Removals Plan,Site Plan
The legal descriptions for the new and vacated drainage and utility easements have been reviewed.Our
review with one comment is enclosed with this letter.Legal description revised and previously approved.
2 The pavement patch for the utility installation shall,ata minimum,match the existing pavement section.The
section to be used will be confirmed at the time of construction;wear course to be a minimum of 1-1/2-inches.
The patch joints will be butt joints.See note 12 added to Site Plan-Sheet C200.
3. The plan calls for raising a sanitary manhole and casting.The City requires a minimum of 2-inches of rings and
maximum of 8-inches of rings.Clarify the details of adjusting this structure. ::ne revised Existing Conditions-
Sheet C100 and Utility Plans-Sheet C500.
4. Include ADA compliant pedestrian ramps where the trail crosses the driveways. Proposed bituminous
trail to be flush with pavement,truncated domes added.See revised Site Plan and Details.

Sheet C300-Grading Plan
1. Refer to the stormwater comments below.The Owner will need to comply with all related
requirements of the N PDES Construction Stormwater Permit and the Brown's Creek Watershed District.
Noted.
2. Further detail and review of the area north and northwest of the building is needed.There is potential
for poor drainage and standing water if that area is not graded correctly,and the drainage pattern in relation
to the adjacent property and building needs to be confirmed.See revised Grading Plan.
3. Indicate a depth of excavation for the pond expansion at the southeast corner of the site. Pond will be
expanded/excavated 8'from 938'down to 930'.
4. The stormwater facilities proposed for the site including facilities that encroach into the drainage and utility
easements and the City right-of-way,shall be considered private and shall be owned and maintained by the
Owner. toted–stormwater facilities to be maintained by owner.
5. Include all applicable City Detail Plates.See Detail sheets C6W-C602.
Sheet 400—Erosion Control Plan
1. Show soil stabilization along the northern side of the site.See revised Erosion Control Plan.
2. The legend shows turf reinforcement mat,clarify where this is being used. Not used,removed from
Erosion Control legend.
3. Include applicable City Detail Plates. See Detail sheets C600-C602.
4. The City reserves the right to require additional erosion control measures as determined/necessary during
the construction of the project.Noted.
Sheet C500-Utility Plan
1. Connections to the City's sewer and water systems shall be per City standards and detail plates.
Note 19 on Utility Plan.
2. The City or its designee will be present for water and sewer service connections to the City's systems.

3. The proposed wet tap in Memorial Drive will need to be coordinated with the City's Public Works
Department.See Note 21 on Utility Plan.
4. The watermain,hydrants and sanitary sewer that serve the site will be private and maintained by the

Storm Sewer/Storm Water Management
1. This property is part of the Kern Center pond drainage area.As part of the agreement with Browns Creek
Watershed District,all stormwater management requirements are provided within the drainage area to
the Kern Center pond.
2. Please provide storm sewer calculations that show the conveyance,at a minimum,of a 10-year storm
event in the parking lot storm sewer system.See attached Storm Sewer Design Spreadsheet for sizing
3. A sump manhole with skimming structure should be placed at the last catch basin manhole of the storm
sewer system prior to entering the Kern Center pond to provide pretreatment.See revised structure 201
on Utility Plan.
4. The proposed new storm sewer facilities on site shall be considered private and shall be maintained
by the Owner,except for the ditch within the relocated drainage and utility easement.See note 23 on
Utility Plan.
5. The project is subject to review by the Brown's Creek Watershed District.The City shall be provided a
copy of the permit approval letterfrom the BCWD and any required maintenance agreements.
General Comments
1. The City reserves the right to inspect the construction of the project.The City or its designee shall be
present for water and sanitary sewer service connections.See note 24 on Utility Plan
2. The Owner will need to comply with all requirements of the NPDES Construction Stormwater Permit
and the Brown's Creek Watershed District.The City reserves the right to require additional erosion control
measures as determined/necessary during the construction of the project.
3. Review of issues related to parking,lighting,landscaping or the buildings is by others.
4. The Owner shall be responsible for obtaining all required permits.
5. Utility construction and improvements in the right-of-way need to conform to the City's Standard Detail Plates.
Include all applicable City detail plates on the plans.The City's details can be provided upon request.

Plant Schedule
The final plans must include the following items:
1. Quantities for each variety of tree/shrub/perennial in addition to the total quantity in the Plant
Schedule.
2. Include the symbols used in the Landscape Legend also in the Plant Schedule. Noted.
3. Include the spacing for Coniferous Shrubs, Deciduous Shrubs and Perennials in the Plant
Schedule (revise X'-X" O.C. and X" O.C.). Noted,
4. I recommend replacing Malus 'Red Splendor' with a disease resistant variety such as Ma/us
'Prairifire' or another species such as Syringe reticulate 'Ivory Silk'. See revised Landscape
?lan Sheet L100.
Landscape Requirements
The locations chosen for most plants are appropriate. The only exception would be the placement of the
trees in the parking lot islands.The trees included in the plans for the islands can grow to be very large at
maturity(25' spread x 25'tall) even though they are ornamental trees.The parking lot medians typically will
not allow enough rooting space for these trees to grow to a healthy maturity and will create traffic visibility
conflicts in the future. I recommend replacing the ornamental trees that will be larger at maturity with
ornamental grasses or ninebark shrubs (both are low maintenance). See revised Landscape Plan Sheet L100.
Landscape Details (Sheet Number L101)
Planting Notes/Planting Detail
The landscape notes and planting detail submitted is acceptable. Please include the following items on the final
plans:
1. All trees must be landscape grade trees of good form and structure not Park Grade trees.Trees with
poor form will be more prone to storm damage and failure in the future. Please include this in the
planting notes.
2. Burlap,wire and all rope must be removed from the top 12" of the root ball on all trees.
3. All trees must be straight and not need staking after planting. Contractor shall stake trees only if
the tree(s) begin to lean within the guarantee period, please include this in the final planting notes.
4. The landscape contractor is required to plant trees and shrubs according to the approved
planting detail diagrams and landscape notes submitted with the final approved landscape plan.
This will be checked after planting and any deviations from the plans must be corrected before the
landscaping will be approved by the City,this needs to be noted in the final plans.
5. Contractors doing any type of tree work must secure a City of Oak Park Heights Tree Worker's License
for 2020 from the City of Oak Park Heights Building Department.All required documents must be
submitted with the application.
6. The City Arborist will conduct an Initial Landscape Inspection after all plants/trees, mulch,ect. have
been installed.
7. The City Arborist will conduct a Final Landscape Inspection typically(1)year after installation, at the
end of the plant guarantee period. For Final Acceptance of Installation and Landscape Plants the

specified planting procedures must have been followed and all plants must be alive and in good
condition at the end of the plant guarantee period.
