OA Park 1!eights, 15431 58 Street, MN AllPhase Companies, Incorporated, (AllPhasc) will he on -site during the excavation of ibotings. We will evaluate for the presence of contaminated sons including petroleum in the soils. 1. WORK SCOPE ITEM AllPhase Companies, Incorporated, (AllPhase) proposes the thllowing Work Scope: • Screen soils every 2 vertical feet during test pits, • Evaluate soils for the presence of contamination and evaluate soil types • Document field information and screening results, 4 Man hours of Field Evaluation during test pit © $70/hr OVM Field screening Device S125/day Soil Testing 3 DRO & GRO sample (ror oil/gasoline based products) [j $80/sample Documentation and reporting 4 man hours @ $751hr t If contamination is present additional expenses will be incurred. If Laboratory analyses of soils are required additional expenses will be incurred. Field screening of soils are included in this proposal. ACCEPTANCE AllPhasc will undertake this proposed scope of service for an estimated cost of approximately 51, 1 85.00. This proposed Work Scope and associated Cost Estimate is based on our knowledge of the project site conditions. Payment is due upon completion of the job unless other arrangements are made. Interest of 1.5% will he charred per month on delinquent accounts. If payment is not received when due, A 1 ]Phase may take any steps necessary to enforce its rights under this agreement. Customer agrees to pay all costs incurred in collection with any delinquent account, including attorneys' fees, whether a lawsuit is commenced or not. The limit of liability, to AllPhase Companies, Inc., of this portion of the project is limited to $ i 00,000.00.
