2. A copy of the Property Tax Statement for the Outlot for taxes payable in 2040;
3. 3 copies of a full -size survey of outlot A Brackey Addition, dated 111211100,
prepared by Westwood professional Services, reflecting the minor subdivision for
which we are applying.;
4. 20 copies of the survey on 11" x 17" paper;
5. My check payable to the City in the amount of $450 for the Base Fee and Escrow
Amount.
