I. LIMITED CONDITIONAL USE PERMIT FOR: Human Services, Inc., Circle of
Friends Program.
All uses. shall be subject to the following conditions and/or
restrictions imposed by the City Council of the City of Oak Park
Heights.
1. HSI will operate the Cicle of Friends adult day care program out of
leased space within this building subject to the following conditions:
A. The Circle of Friends Program will be operated out of this space
during daytime hours.
B. These permit will terminate on the day following the earliest of the
followin occurances: ex iration of the initial lease of the sublect s ace
relocation of the program from the premises or June 1, 2003.
C. Occupancy of the premises by the Program will only be allowed after
a fire suppression system has been installed and is operational__ asapproved
by the office of the Oak Park Heights building inspector.
D. Occupancy of the space by the program will only be allowed after the
ro ert is connected to cit water and sanitar sewer service.
E. Maximum enrollment into the program shall be limited to those
r in licensure requirements of the Minnesota Department of Human
Services based on 2600 square feet of leased space.
F. Parkin ■ stalls shall be clearl stri••ed to • rovide a total of 76
spaces as illustrated in the attached site plan.
G. The •arkin■ stall ad'acent to the northeast buildin■ entrance shall
be designated for short term(20 minute )parking and one additional disability

III. REVIEW
Pursuant to Order of the City Council of the City of Oak Park Heights,
a periodic review of this Conditional Use Permit is imposed as a
condition of its grant. This Conditional Use Permit shall be reviewed
on June 20, 2000 at the Direction of the City Clerk, which shall notify
the permit holder of the date of the annual review at least ten (10)
days prior to the review hearing.
