Hi Eldon,
Congratulations on the sale of mother and step- fathers home. I am guessing what the closing
agent is looking for from you is the Minnesota Department of Health's file number or certification
of well closure. This would be the document issued upon completion of the MN Dept. of Health
criteria process I mentioned that needed to be followed in my September 2, 2009 email to you.
If you did not contact them for their process, you will not have received this information. I am
attaching a link to a brochure on well sealing from their web page. The brochure also notes
contact information. If you did not contact them, I would do so immediately by calling the St. Paul
District Office number at 651- 201 -4600 or at their toll free number of 1- 800 - 383 -9808.

At this point in time, I have not received any documentation from them that they have certified the
well closure, but I do have the pictures you passed along for the property file. If you would like I
can send them onto someone at the Department of Health, if you can provide me with an email
address after contacting them. Perhaps they can review the photos after speaking with you and
provide you with the information you need.
