The purpose of this letter is to deliver the Information Summary for Comprehensive
Plan Revisions and seven (7) copies of the Comprehensive Plan Amendment and
MUSA Expansion report for the Kern Center in Oak Park Heights. The amendment
proposes a change to its c:jrrent comprehensive plan for the purpose of including the
Kern Center as Planning District 18 and to request a Metropolitan Urban Service Area
(MUSA) expansion for the area in question.

This completed form should accompany each comprehensive plan revision or plan amendment
submitted to the Metropolitan Council for review. The information submitted on this form is
reviewed by Council staff within 10 working days to determine if the comprehensive plan or
amendment is sufficiently complete for Council review purposes.
NOTE: Plan amendment s that are simply "housekeeping" changes, need only supply the
information in Section I of this form.
