I have been advised by Public Works Director Johnson that the above -
referenced addressed is in need of a refund.
May 12, 2003 DDD, LLC paid for building permit #03 -63. The payment of
this permit included amounts for two 1" water meters and one radio. At the
time of issuance, it was determined that two 1"' meters were needed for the
site (1 for the house meter and 1 for irrigation).
As it would turn out only 1 - 1" meter is needed for the project. The meter
issued for irrigation was not issued. I have attached the water meter receipt
for your reference of ECR coding and such in the event that utilities has
established an account with the information. Any account should be
deleted with this information.
The amount of $205 should be refunded to DDD, LLC and returned to the
attention of Steve Continenza at 3686 Layton Ave., Lake Elmo, MN 55042.
Kindly copy me the refund check and any note that you may send
forwarding this refund at such time the refund is issued.

AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be
performed according to : (1) the conditions of this
permit; (2) the approval plans and specifications; (3) the
applicable city approvals, Ordinances, and Codes; and,
(4) the State Building Code. This permit is for only the
work described, and does not grant permission for
additional or related work which requires separate
permits. This permit will expire and become null and
void if work is not started within 180 days, or if work
is suspended or abandoned for a period of 180 days any
time after work has commenced. The applicant is
responsible for assuring all required inspections are
requested in conformance with the Minnesota State Building
Code.
