AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1) the conditions of this permit; (2) the
approval plans and specifications; (3) the applicable city
approvals, Ordinances, and Codes; and, (4) the State Building
Code. This permit is for only the work described, and does
not grant permission for additional or related work which
requires separate permits. This permit will expire and become
null and void if work is not started within 180 days, or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

23 New Life Contracting, Inc. is in compliance with installation and application of Ice and Water
Barrier by —
Underlayment & Ice and water protection: Entire state of Minnesota requires special eave protection
an ice barrier that consists of at least two layers of Underlayment cemented together or of a self - adhering
polymer modified bitumen sheet, shall be used in lieu of normal Underlayment and extend from the cave's
edge to a point at least 24 inches (610 mm) inside the exterior wall line of the building. The code in
Minnesota requires this special waterproof shingle Underlayment at the eaves or lower edges of the roof.
New Life Contracting, Inc. is responsible for installing such underlayment extending up to a minimum of
24" from exterior walls.
Ice and Water Shield In Valleys: Valleys shall have a 36- inch -wide (914 mm) Underlayment of either
one layer of Type I Underlayment running the full length of the valley, in addition to other required
Underlayment, or a self - adhering polymer modified bitumen sheet complying with ASTM D 1970.
