Dear Eric:
We have reviewed the site development plans received for Oak Park Senior Living, Phase 1, as submitted by
Folz, Freeman, Erickson, Inc. on October 12, 2011. Following are our comments and /or recommendations.
Sheet 2 — Master Plan & Traffic Analysis
1. The traffic analysis has been reviewed and the trip calculations have been verified. The trip
generations at the peak hours (AM and PM) are fairly low and are not of concern.
Sheet 4 — Site Drainage /Grading Plan /Snow Removal
1. The existing storm sewer in the southwest corner of the site needs to be shown on an existing layer.
2. Show the redefined drainage and utility easement for the 21" diameter storm sewer between Building
1 and 2 on both Sheets 4 and 5.
3. The 21" diameter storm sewer shall be reinforced concrete pipe (RCP).
4. All storm sewer design data needs to be displayed (rims, inverts, structure type and sizes, pipe
slopes, and material).
5. The existing catch basin located just north of Building 2 shall be verified as being a sump catch basin
manhole for sediment and debris removal, prior to discharging into the northern pond. If this catch
basin structure is to remain, the former pipe opening to the south requires bulkhead.
6. Another catch basin needs to be considered within the 46 stall parking lot for Buildings 1 and 2. The
entire western parking lot sheet drains across the driveway entrance.
Sheet 5 — Utility Plan
1. Rather than abandoning the existing sanitary and storm sewer as labeled, the pipe and structures
need to be removed.
2. The existing hydrant located in the driveway entrance to the Building 1 and 2 parking lot needs to be
relocated.

Sheet 6 — Lighting Plan
1. Verify proposed light poles do not conflict with existing and proposed utilities.
Preliminary Plat
1. Identify "NEW DRAINAGE EASEMENT" for storm sewer between Buildings 1 and 2 as "NEW
DRAINAGE AND UTILITY EASEMENT ".
2. Outlot A needs to show the 10' wide drainage, utility, and trail easement on the north side of 58
Street North.
General Comments
1. The scales shown on the 11 "x17" size plans are not accurate and need to be shown correctly.
2. Several City standard detail plates are provided as attachments.
3. Records plan drawings showing as -built information need to be provided at completion of
construction.
