RE: Oak Park Heights — Oak Park Senior Living Phase I — PUD Amendment

Dear Mr. Nolde:
The City deemed your application materials received on September 20, 2011 for an
amendment to the Planned Unit Development (PUD) approvals for Oakgreen Village
located at Nova Scotia Avenue North and 58 Street. The application consists of a
request to amend the PUD to allow for a reduction in the number of approved
townhome units and to construct brownstone senior living buildings. With this letter we
are notifying you that the application is not complete. The City will deem it complete
with receipt of the following items by 3:00 pm, September 28, 2011, should these items
not be received by such date and in total, the application will be rejected as incomplete:
1. Provide an ownership lien and encumbrance report for the property included in
this application.
2. Provide a narrative and documentation on the proposed changes and transition
to a revised Development Agreement and the Homeowners Association
Documents.
3. Provide a written update as the status of the October 4, 2011 neighborhood
meeting including projected date of written notification to surrounding property
owners and how (and by whom) that meeting will be executed.
4. A Preliminary Plat.
5. Detailed utility plans separated from the colored site plans.
6. Full scale copies of the entire plan sheet set, including utility plans
7. Dimensioning of all of the site plans and architectural plans.

8. Include a table indicating the square footage and number of bedrooms tied to
each of the floor plans.
9. Provide calculations of overall project density including the recently completed
Oakgreen Commons. Calculate the square footage of land area per dwelling unit
for each of the three phases and as a total.
10. Provide a copy of the approved plans for the Oakgreen Village project with the
proposed total number of units that were to be constructed at build out.
11. Indicate on the plans how access will be provided to the garages of the four units
at the south west corner of the project. Will these units be constructed as part of
this Phase I and will they be identical to the existing four -plex immediately north?
12. Provide a lighting plan with photometrics, images of the fixtures and specification
sheets on all of the fixtures to be used. If any of the fixtures to be used are of a
residential style and not full cut off, please specify at this time.
13. Provide a specific signage plan for any wall or monument signs to be used for the
development.
14. Provide a snow storage /removal plan.
15. Indicate if trash /recycling is to be stored inside the buildings or if outdoor storage
facilities are to be used.
16. Provide mechanical detail plans and screening for any outside units.
17. Include on all of the building elevation plans details on the types and colors of
building materials. A materials board shall be required.
18. A graphic perspective of the development from various locations shall be
required.
19. Any additional landscape details required by the City Arborist.
All plans and documentation as required by this letter shall be submitted directly to Eric
Johnson, City Administrator by 3:00 pm September 28, 2011, in hard copy and CD
format. No email submissions shall be permitted nor will be accepted. Do not email
information, respond as specifically directed.

It is important to note that Staff reviewed the application materials at a meeting on
September 21, 2011. The architecture of the buildings and the lack of variety in style
will likely need to be addressed as this project proceeds, being they are not consistent
with the City's Design Guidelines and we encourage you to carefully consider the
architectural look of these facilities.
The buildings are very horizontal in design with few architectural features to break up
the long facades. We would additionally ask that you provide some alternate plans to
break up singular buildings and provide some different features and or colors from
building to building.
We invite you and your representatives to a meeting with City Staff to discuss this
application on September 28, 2011, 10:00 A.M. at City Hall.
If all of the application materials are submitted as requested, the Planning
Commission would consider the application at a meeting scheduled for October
13, 2011 at 7:00 P.M. in the City Hall Council chambers. Should there be any
lacking data, the City will reject this application in total.
Pursuant to Minnesota Statutes 15.99, Sub. 3(f), the City of Oak Park Heights is
herewith extending the period of time for agency /city review of the application for an
additional 60 day period (for a total of 120 days from the date of a complete application)
inasmuch as the City has determined that additional research and study by the City staff
relative to the complexity of the project as proposed will be required.
