PO Box 650043 Dallas TX 75265 -0043 Ref: Invoice for 5472 Stagecoach Trail N., Oak Park Heights, MN 55082 Property ID #03- 029 -20 -42 -0002 To Whom It May Concern, Upon receiving complaints regarding the condition of the property you own at 5472 Stagecoach Trail N., the City of Oak Park Heights notified you that the property had several zoning code violations. These violations were noted in a phone conversation and e-mails with your listing real estate agent, Mr. Andy Skwiot, Realtor GRI, on June 14, 2011. Mr. Skwoit was directed to bring the property into compliance within ten days of that date. This letter is to serve as an invoice for the City's administrative time spent addressing these code violations. City's Building Inspection Department: Site investigations, letters, purchasing of locks, posting of notices, and follow up inspections for the yard cleanup. Total of 2 hours @ $60 per hour for a cost of $120 Purchasing of locks from Lowes for a cost of $12.69 Total cost due the City of Oak Park Heights is $132.69. Please note that if this bill is not paid to the City of Oak Park Heights, the costs shall be assessed to the property for tax collection in 2012.
