AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1) the conditions of this permit; (2) the
approval plans and specifications; (3) the applicable city
approvals, Ordinances, and Codes; and, (4) the State Building
Code. This permit is for only the work described, and does
not grant permission for additional or related work which
requires separate permits. This permit will expire and become
null and void if work is not started within 180 days, or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

Subject: Stillwater Motor - New water meter
I had a request for a meter for Stillwater Motors. I pulled the account information and they have an existing 1 inch meter
that is used to measure their well flow for sewer billing. They told me they have permit number 2005 -00216 for plumbing
but they did not know if a meter was included in the permit. I asked some questions and I received a fax from Mark
Ohmann from Bruce Nelson plumbing showing the new fixtures to be placed on the new city water. They have a lot of
flush valves but also many little use connections. They will need a 2 inch compound meter for the new service. It is my
understanding their well will continue to be used for the car wash and the existing 1 inch meter will continue to be used for
metering the water so we can charge for sewer on this line.
An inspection will be needed to verify that the well water is physically separated from the city water lines. They will also
need to have additional inspections and testing results in hand before piping that was used on their well system can be
used for city water. They were talking about phase 2 and three where they will tie in and abandon the existing pipes used
on well water. I told them that all piping will need to have bac -t results before we will turn on City of Oak Park Heights
water.
Julie, can you let me know if they requested a new meter and service and if permit fees were collected for meter and
inspections? Let me know if we need to draft a complete list of meter fees, I just gave you the raw numbers. Also need
to verify they have paid any required WAG and expanded SAC charges before I order a new meter.

Just got a call from Garth at Langer Construction. He has generated a check for $1740.54 (I don't know why it
has an extra $30) to cover the meter and inspections. He said he is sending the check with BN to pick up the
meter and permit. He said the city could refund the difference if he did not have the right amount for the check as
he wants to avoid any delay. He is also having Miller Excavating call me regarding the water tie in they did to the
existing 6 inch water line. I have no record of this being done and did not inspect the movement of the hydrant or
the disinfection of the new fittings that was supposedly done. I'll ask Jeff if he had anything at Stillwater Motors
with Miller. I know he had some involvement with Lake Elmo bank and Miller but don't remember a Stillwater
Motor inspection. Garth also said he did not know if any trace wires were installed. Issues have been raised and
I'm a little concerned the city has not had the opportunity to verify some of the work done on the mains or
incorporate the trace wire.

ON HOLD. They are still arguing about who is supposed to pay for the meter. Bruce Nelson (BN) does not want
to pay for the permit until they have a commitment from the general or the owner on who pays. Sorry, they said
Rick should not have called me this morning to get this done. Does BN have any permit going right now for the
work they are doing?

Subject: Stillwater Motor new water meter
Bruce Nelson (or someone representing Langer Construction) has a check coming in for inspection and meter
payment for the 2 inch compound water meter for Stillwater Motors. I have placed this heavy meter in the hall
outside Lisa's area for the contractor to pick up. They are supposed to stop up front and pay for and sign the
permit before meter pick -up. Total Due for Permit #2006 -25 $1,710.54. This includes $1500.54 for the following
meter and parts:

MXU radio ID 12420612
The MXU, flange and bolts are in the box with the meter. I will be out of the office the next 3 days so thought I'd
better leave this ready to go.
