The intent of this Updated Final Inspection List is: •to identify critical items of Work that require repair, replacement or completion. •to identify damage(s) to Work in -place at the time of the Final Inspection as being the sole responsibility of each applicable and respective Contractor to remedy. •to identify items of Work that require immediate completion so as to allow the Owner to properly occupy the portion(s) of this building identified herein. Note that incomplete and in- progress items of Work are not fully identified in this document. The Construction Documents provide that information. It is the Contractor's responsibility to ascertain the scope, extent and specifics of the items of Work remaining to be completed under the terms of the Contract. The term, "Provide" utilized herein is defined as: to furnish and install all materials, equipment and labor so as to complete each item of Work in accordance with the Contract for Construction. Completion of each and every Updated Final Inspection List item of Work by the Contractor is one of the Contractor's contractual obligations necessary to be achieved in order to receive final payment. It is the Contractor's contractual obligation to complete all Work of the Contract. Complete all Work including the items listed herein and send notification to the Architect's Office of that completion so as to arrive at the Architect's Office no later than October 8, 2010 so as not to deter, hinder and otherwise adversely affect the operations of the Owner and with each respective corrected item of Work identified by the placement of a check mark (V) or the name of the organization that completed the correction of that item of Work in the page margin adjacent to each such item of Work (thus, specifically indicating that each such item of Work is complete). Verify that each respective item of Work is actually complete before asserting to the Architect of its completion. All fees and costs for each additional Field Review and /or Inspection and all fees and costs for the preparation of each and every additional Final Inspection List and /or Updated Final Inspection List will be charged to the Construction Contract at the Contractor's expense. Contractor's Certification of Completion: The Contractor shall complete the information below and shall provide a personal signature where indicated below. Provide additional Finish Hardware per 08710 1.9A and 1.9B. Site Provide a written statement that the Site Work is completed and is ready for a Final Inspection. That statement must include a list of uncompleted items. • Paint hinge bands of each gate (that allow swinging motion) at Trash Enclosure. Color shall match support posts onto which these hinge bands are attached. • Vertical posts of Trex fencing exhibit horizontal discolorations: Provide written verification from manufacturer as to when those discolorations will fade so as not to be visible. • Seeding installation of site areas north and east of City. Buildings (City Hall, Public Works Building and Pump Building) Provide a written statement that the Exterior Work and Interior Work of the City Hall, Public Works Building and Pump Building is completed and is ready for a Final Inspection. That statement must include a list of uncompleted items. Building Exterior (City Hall) City Hall: • Provide wall- mounted exterior canopy each above Opening: 003B and 023C. • Replace creased /bent /damaged roof panel located on east downslope area below east clearstory window (this clearstory window serves Room 141). Building Exterior (Public Works Building) Public Works Building: • Provide continuous sealant at jambs and heads of exterior door frames: exterior and interior sides. Building Exterior (Pump Building) Pump Building: • Remove bituminous roofing drips from prefinished roof edge coping. • Remove old sealant and apply new sealant to exterior louver on west wall. • Provide sealant at piping that extends from interior through east exterior wall. Building Interior (City Hall) General Items • Provide training to Owner in use of all equipment, assemblies and systems: Interior and Exterior Lighting Controls, Photoelectric Sensors, Energy Management System, CO /NO Exhaust System, Fire Alarm System, Audio Visual Systems and others: Provide written evidence for each type and instance of training that Owner has received. • Patching and repairs shall be neatly accomplished and indistinguishable from what the original finish would look like if properly accomplished at the onset of the Work. Thus, when repairing damages to interior surfaces (such as walls, base, soffits, fascias and ceilings, paint the entire surface such that the following are not visible after the work is completed: repairs, patches, brush marks, roller marks. Rooms General Items Room 015 Mens • Remove foreign materials from rim of escutcheon at each grab bar in Toilet Rooms. • Provide sealant at all under - counter areas at countertops where brackets are cut in and where balancing sheet is disturbed or has been removed. • Adjust locker door lock operation that currently is stiff /sticky at: Locker #6. • Provide sealant at electrical receptacle cutout within locker unit at: Locker #2 and #11. Room 016 Womens • Remove foreign materials from rim of escutcheon at each grab bar in Toilet Rooms. • Provide sealant at all under - counter areas at countertops where brackets are cut in and where balancing sheet is disturbed or has been removed. • Provide sealant at electrical receptacle cutout within locker unit at: Locker #1, #3 and #4. Repaint all eight sections of this horizontal band. • Repair and refinish ceiling soffit surfaces where sprinkler system water leaked at west wall. Room 105 Men • Remove foreign materials from rim edge of escutcheon cover at each grab bar in Toilet Rooms. • Provide sealant at all under- counter areas at countertops where brackets are cut -in and where balancing sheet is disturbed or has been removed. Room 120 Conference • Provide plastic laminate strip at vertical edge on north side of countertop. Room 126 Break Room • Repaint entire north and west walls and especially at unfinished patched area adjacent to baseboard radiation. Room 137 Office • Repaint entire South wall that exhibits uneven and streaked paint application. • Repaint entire north and west walls and especially at unfinished patched area adjacent to baseboard radiation. Room 164 Locker Room • Provide escutcheon at pipe that penetrates ceiling along east wall. • Paint north wall especially at and below lavatory. Building Interior (Pump Building) Rooms General Items • Remove debris, marks and foreign materials from floor, wall and window surfaces. Openings • Wood door faces and edges are rough to touch (sand embedded in finish ( ?): smooth down without ruining finish. All openings are affected. Repair required at each door leaf. Opening 100A • Remove foreign materials from faces and edges of aluminum framing at door jamb. • Repair abraded and scratched aluminum framing at head of opening above sidelight. Opening 105 • Repaint scratched frame in its entirety. Opening 164 • Provide rubber button bumpers within edge of frame. City Hall Building Exterior • Provide protective cage around Temperature Control System sensing device(s) located on north wall of building above Opening 003B. Building Interior Room 112 Police Chief • Repair or replace loose baseboard radiation end cover. Electrical Electrical Engineering Consultant's Final Inspection List Attached is the "Final Mechanical/Electrical Inspection Report for Oak Park Heights (dated May 7, 2010). Complete all Electrical Work and provide notations and signatures that state that each item of Work is completed. Hierseman/Dan Moulton M & E COMMISSION NUMBER: 12908 INTRODUCTION The purpose of this visit was to observe the Mechanical and Electrical installation for the Oak Park Heights City Hall. This report shall in no way relieve the Contractor of responsibility for completion of any and all work according to the terms of the Contract. The City Hall building is 98% complete or more. This list does not include obvious items to finish like controls, insulation, etc. This list does not include the Public Works Building. PUNCH LIST GENERAL 1' U MS Date Completed 1. Balance air and water flows and submit report. 2. Operation and maintenance manuals. 3. Owner instructions. See Specification. 4. Extra filters for all air handling equipment. See Specification Section 15801, 3.01. 5. Clean dust from all equipment and controls. 6. Field testing and verification documentation of electrical equipment and systems is required, but will not be done until later. Submit testing documentation. 1of3 MECHANICAL TTEMS 1. Provide final connection of water to humidifier in duct. 2. Provide relief valve for boiler and pipe to floor. 3. Provide corner piece of radiation in room 142. 4. Provide radiation in room Break Room 126. 5. Miscellaneous pieces of radiation and some trim pieces are missing. Install when available. 6. Some of the radiation has chips in the paint and will need to be touched up. 7. Install radiation in Council Chambers. 8. Verify all plumbing fixtures are operating with sensors and have correct flow. Set hot water temperatures to required settings. 9. The following is the temporary conditions issued on September 17, 2010 to allow the system to operate with reduced cooking capacity until the additional wells are completed after the existing building is removed. Temporary air flow to the lower level while second phase of the geothermal well system is being completed. This will provide exhaust for the locker room and evidence rooms and provide air flow in the corridors to provide a positive pressure against the garage. 1. ERV -1 to operate as designed. 2. Provide a 20 x 12 bypass duct from the 22 x 14 exhaust to the 18 x 12 supply above room 019. 3. VAV box 001 to be full open. 4. Cut a 10 x 10 opening in the 46 x 14 duct above the ceiling in Hall, 024 and Women's Locker room, 016. Provide a 24 x 24 eggcrate in the ceiling of the Hall, 024 and the Women's Locker room, 016. 5. Connect a 20 x 10 bypass duct from 22 x 14 duct in 019 to 16" round duct in 019 (from VAV 002). Close and shut down VAV 002. 6. Exhaust fan EF -10 will be off. 7. When the geothermal system is complete, remove the bypass ducts, the (2) eggcrates and return the VAV box controls to normal. Patch ductwork as required. Operate EF -10 as designed. 8. To reduce the extra cost of balancing the two VAV boxes and EF -10 will not be balanced until after the system is back to normal. ELECTRICAL ITEMS 1. Clean light fixtures. There is debris inside the direct/indirect light fixtures and the glass in the council chambers light fixtures is dirty. 2. Most of the systems are incomplete and in progress. A follow -up site observation will be required to verify completion. 2 of 3 3. The voice/data outlets are both blue. Provide gray for telephone and white for data. Refer to Specification Section 17741, 2.05,G. 4. Installed receptacles are white. Specified is gray. 5. Specifications do not specify the quantity of amplifiers, preamps, etc for the public address and music systems. The quantity required is as necessary to operate the system. The door bell for the Police Reception is to sound in rooms 102, 116, 110, and 122. The door bell for the City Hall Reception is to sound in rooms 125, 126, 130, 133, 134, and 141. If the Owner signs off on different locations for sounding of these door bells, at least provide separate wiring for future operation as indicated. The Landscape Subcontractor has applied seeding and protective netting to areas north and east of the new City Hall. 2. The hand broadcast rate for the seed mixtures recommended by Prairie Restoration is 15 -16 PLS /acre. 3. The seed mixtures proposed by the Landscape Subcontractor (and surely applied) is 12 and 13 PLS /acre — that is too low. 4. At the applied areas that currently have protective netting, the Landscape Subcontractor must guarantee proper coverage and must, otherwise, come back and top dress those areas that don't exhibit a thick enough growth. 5. Edges at concrete curbs /gutters are too high, not neatly applied and slopped over the gutter edges in spots. It is expected that they will be subject to premature edge deterioration over a short period of time. 2. "Bird bath" ponding areas are still present notwithstanding whatever effort was applied by the Subcontractor. Solutions such as surface patches or cut -outs and reapplication of the wear course and. that would have visible joints would not be not acceptable. 3. Exposed aggregate in the wear course visible at paving surfaces are not acceptable. It is expected that those surfaces are subject to premature edge deterioration over a short period of time. At Wednesday's meeting, a representative from the Bituminous Paving Subcontractor (Kirk Langumof Hardrives) indicated that an aesthetic improvement to the current conditions can be achieved by application of aggregate fines, heat and a roller application. If he wants to try this, it should be applied to all paved surfaces. Also there are irregular joint areas and I didn;t hear how those araes would be improved. Today I asked Braun Intertec for a Proposal to provide the following services: visual inspection of the paving bituminous installation, core drills through the paving and paving improvement recommendations. We understand there is some concern about the condition of the bituminous pavement based on observations of the surface made by you, which includes areas of possible aggregate segregation (reported as coarse aggregate exposed in areas of the surface) and several depressions /birdbaths. We also understand that thickness and density of the bituminous surface have not been measured. Scope of Services The following tasks are proposed for our evaluation. If unfavorable or unforeseen conditions are encountered at any point during the completion of the tasks that lead us to recommend an expanded scope of services, we will contact you to discuss the conditions before resuming work: • Visual assessment by a pavement engineer. In order to form an opinion about the condition of the lot and provide recommendations for further exploration and possible repair, we propose that a pavement engineer visit the site and visually assess and document the lot's appearance. • Coring of the bituminous surface. We propose to extract three (3) cores, 6 inches in diameter, in each of the light- and heavy -duty areas of the parking area in order to obtain samples for laboratory testing if deemed necessary based on the visual assessment. We will patch the coreholes with a bituminous cold -patch mix. • Completion of Dynamic Cone Penetrometer (DCP) tests. If coring is completed, we will also perform two (2) DCP tests in selected corehole locations in order to evaluate the stiffness of the supporting soils, including the aggregate base, subbase and subgrade. This information will assist us in evaluating the cause of the various distresses that may be present. ■ • Providing engineering and environmental solutions since 1957 City of Oak Park Heights Proposal SP- 09- 01319A September 21, 2010 Page 2 • Laboratory testing of the extracted samples. We propose to measure the bulk density and thickness of any pavement cores that we extract. These bulk densities will be compared to a Rice specific gravity in order to determine relative density, which we will determine for each pavement type (light -duty and heavy -duty) with the core sample material. We will also perform a gradation of the extracted aggregate for each pavement type. • Letter of opinion providing recommendations. Based on our visual assessment, and (if performed) the results of DCP and laboratory testing, we will provide a brief letter of opinion regarding the condition for the lot, and will include recommendations for repair of the lot as necessary. One original letter and one copy of our letter will be submitted to you. At your request, additional copies can be prepared for other project team members. Cost We will furnish the services for the pavement engineer's assessment and opinion described in this proposal for an estimated fee of $1,101. We estimate that coring, DCP testing and laboratory testing will cost an additional $1,134. A tabulation showing total hourly and /or unit rates associated with our proposed scope of services is attached. Our work may extend over several invoicing periods. As such, for work that is performed during the course of each invoicing period, we will submit partial progress invoices. Schedule We anticipate the field exploration can begin within approximately one week of written authorization and will take one day to complete. We anticipate we can submit the results of our evaluation report approximately three weeks following authorization, but will make every effort to accommodate your schedule assuming the possibility of additional construction later this year. If our proposed scope of services cannot be completed according to this schedule due to circumstances beyond our control, we may need to revise this proposal prior to completing the remaining tasks. General Remarks We will be happy to meet with you to discuss our proposed scope of services further and clarify the various scope components. We appreciate the opportunity to present this proposal to you. It is provided in duplicate so the original can be retained for your records and the copy can be signed and returned to us. Please return the signed copy in its entirety. The proposed fee is based on the scope of services described and the assumptions that our services will be authorized within 30 days and that others will not delay us beyond our proposed schedule.
