Dear Mr. Peterson,
I was contacted by the property owner of the above - referenced address to schedule inspection of work done
by your company at the home. I informed the homeowner that I had no permit issued for work to the
residence and met with them at the residence. At the time of my visit, the homeowner shared documentation
that itemized work and charges to them from Custom Renovations LLC, including charges for permits and
survey work.
At this time no permits have been applied for or issued for the remodeling work to the upper and lower
bathrooms, which included a window installation, shower insert(s), plumbing and fan. Nor has a permit been
issued for the lower level remodeling work, wherein the furnace ductwork was enclosed has not been vented,
though one wall is marked for a vent. It is my understanding that three doors were also installed. At the time
of my visit, I tried the doors and found that none of them closed and locked properly. Per the paperwork
shared with me a fence has also been contracted for.
Please complete the enclosed permit applications for work done at this home and return them to me along
with verification of your MN State Residential Contractor License and Lead Certification. Upon receipt I will
process them and contact you to arrange for an onsite visit /inspection of the work so that code compliance
may be verified.
Electrical permits are required by the State of Minnesota through the Department of Labor and Industry. If
you have any questions as to this, you may contact them at the general number of 651.284.5026. I have
notified our Regional Electrical Inspector of this project.
Oak Park Heights requires a remodeling permit for the bathroom remodeling, rocking work in the basement
and door installation. We also require a separate permit for plumbing remodeling, work which is to be
performed by a MN licensed plumber. The plumber who did the work should be noted on the permit
application and verification of licensure provided.

Lastly, at such time you are prepared to install the fence, you are required to pull a fence permit and
provide a copy of the survey done for verification of setbacks.
If there is other work that has been done or is planned to be done, that I have not covered above,
please included it upon your permit application(s) as appropriate.
A "Stop Work" notice has been left at the home and no work is to occur until such time permits have
been issued by the City of Oak Park Heights.
Submit your permit application(s) to my attention as soon as possible, but no later than Thursday,
September 28, 2012. You may fax, mail or email your applications and accompanying documentation.
