Box 119 Stillwater, Minnesota 55082 RE: Oak Park Heights — Oak Park Senior Living Phase III — Proposed Memory Care - PUD Amendment FILE NO: 236.02 —13.01 Dear Mr. Nolde: The City has received your application materials on March 13, 2013 for an amendment to the Planned Unit Development (PUD) approvals for the Oak Park Senior Living Phase III proposed memory care building located near Nova Scotia Avenue North and 58 Street. The application consists of a request to amend the PUD to add a 30 unit, two story addition to the existing Senior Living Facility and to relocate an existing three unit townhome building. With this letter we are notifying you that the application is not complete. The City will deem it complete with receipt of the following items by April 3, 2013. Should the following items not be received by such date and in total, the application will be rejected as incomplete: 1. Provide a narrative on the proposed changes. Please clarify in the narrative that the Layout Plan is what is currently being requested with this application. Also, please indicate if you will be asking for any financial /bonding assistance as part of this project. 2. Provide calculations of overall project density. Calculate the square footage of land area per dwelling unit for each of the three phases and as a total. 3. Provide a table reviewing the parking calculations for the current phase and the project as a whole. 4. Include a table indicating the square footage and number of bedrooms tied to each of the floor plans. Tim Nolde March 20, 2013 Page 2 5. Provide a lighting plan with photometrics, images of the fixtures, and specification sheets on all of the fixtures to be used, for both street and building lights. If any of the fixtures to be used are of a residential style and not full cut off, please specify at this time. 6. Provide a specific signage plan for any wall or monument signs to be used for the development. 7. Provide a snow storage /removal plan. 8. Indicate if trash /recycling is to be stored inside the buildings or if outdoor storage facilities are to be used. 9. Provide mechanical detail plans and screening for any outside units. 10. Provide a landscape plan with details required by the City Arborist. All plans and documentation as required by this letter shall be submitted directly to Eric Johnson, City Administrator by April 3, 2013, in hard copy and CD format. No email submissions shall be permitted nor will be accepted. We invite you and your representatives to a meeting with City Staff to discuss this application on April 3, 2013, 10:30 A.M. at City Hall. If all of the application materials are submitted as requested, the Planning Commission would consider the application at a meeting scheduled for May 16, 2013 at 7:00 P.M. in the City Hall Council chambers. If recommended by the Planning Commission that evening, the City Council will review the application at their meeting scheduled for May 28, 2013. Pursuant to Minnesota Statutes 15.99, Sub. 3(f), the City of Oak Park Heights is herewith extending the period of time for agency /city review of the application for an additional 60 day period (for a total of 120 days from the date of a complete application) inasmuch as the City has determined that additional research and study by the City staff relative to the complexity of the project as proposed will be required.
