This document was developed to establish the date of Substantial Completion for the purpose of commencement of applicable warranties and to allow the Owner to occupy or utilize the Work or des- ignated portion. RELATED DOCUMENTS. This document was prepared for use under the terms of AIA general conditions, including AIA Document Azoi, General Conditions of the Contract for Construction, and the general conditions contained in AIA Document Aio7, Abbreviated Owner - Contractor Agreement Form for Construction Projects of Limited Scope— Stipulated Sum. USE OF CURRENT DOCUMENTS. Prior to using any AIA document, the user should consult the ALA, an AIA component chapter or a current AIA Documents Price List to determine the current edition of each document. REPRODUCTIONS. The intent of this Final Inspection List is: •to identify critical items of Work that require repair, replacement or completion. z •to identify damage(s) to Work in -place at the time of the Final Inspection as being the sole responsibility of each applicable and respective Contractor to remedy. •to identify items of Work that require immediate completion so as to allow the Owner to properly occupy the portion(s) of this building identified herein. Note that incomplete and in- progress items of Work are not fully identified in this document. The Construction Documents provide that information. It is the Contractor's responsibility to ascertain the scope, extent and specifics of the items of Work remaining to be completed under the terms of the Contract. F • The term, "Provide" utilized herein is defined as: to furnish and install all materials, equipment and labor so as to complete each item of Work in accordance with the Contract for Construction. o Completion of each and every Final Inspection List item of Work by the Contractor is one of the Contractor's contractual obligations necessary to be achieved in order to receive final payment. It is the Contractor's contractual obligation to complete all Work of the Contract. Complete all Work including the items listed herein as to the building's interior and send notification to the Architect's Office of that completion so as to arrive at the Architect's Office no later than May 14. 2010 so as not to deter, hinder and otherwise adversely affect the operations of the Owner and with each respective corrected item of Work identified by the placement of a check mark (✓) or the name of the organization that completed the correction of that item of Work in the page margin adjacent to each such item of Work (thus, specifically indicating that each such item of Work is complete). Verify that each respective item of Work is actually complete before asserting to the Architect of its completion. All fees and costs for each additional Field Review and /or Inspection and all fees and costs for the preparation of each and every additional Final Inspection List will be charged to the Construction Contract at the Contractor's expense. Provide additional Finish Hardware per 08710 1.9A and 1.9B. Site (Inspection will occur when Site Work is completed and after Contractor provides written notification of completion and written list of uncompleted items). Building Exterior (City Hall and Public Works Building) Provide a written statement that the BuildinExterior is completed and ready for Final Inspection. A Final Inspection will occur later this week and a Final Inspection Report will be issued. P P P Building Interior (Public Works Building) Provide a written statement that the Public works Building's Interior is completed and ready for Final Inspection. A Final Inspection of the Public Works Building will be conducted this week. Building Interior (City Hall) Provide a written statement that the City Hall's Interior is completed and ready for Final Inspection. General Items • Provide the specified window blinds throughout facility: Specified blinds were to have 1" blades. Blades provided are 3/4" and are not acceptable. • Provide training to Owner in use of all equipment: Interior and Exterior Lighting Controls, Photoelectric Sensors, Energy Management System, CO /NO Exhaust System, Fire Alarm System, Audio Visual Systems and others. • Clean all surfaces: Interior of building is still very dusty (fingerprints on glass, hardware, frames, doors, cabinets and equipment). • Remove smudges, dirt and foreign materials from stainless steel surfaces: at Elevator frames, doors and interior of cab and at toilet accessories • Provide at all exterior windows clear silicone sealant at horizontal joint between solid surface sill and wood window sill. • Provide Interior Signs at Council Chamber Dais. • Provide all interior signs. • Remove foreign materials from each grab bar in Toilet Rooms. • Markerboards: Provide one (1) magnetic paper holder at each Markerboard. Provide two (2) sliding map hooks for each 48" of length at each Markerboard Unit. • At countertops: seal at all undercounter areas where brackets are cut in and where balancing sheet is disturbed or has been removed. • Provides keys to Owner for: Entrances, Openings, Millwork/Casework, Toilet Accessories, Electrical • Cabinets, Fire Alarm Cabinet and others. Thus, when repairing damages to interior surfaces (such as walls, base, soffits, fascias and ceilings, paint the entire surface such that the following are not visible after the work is completed: repairs, patches, brush marks, roller marks. Rooms General Items • Drawing Sheets A2.1 and A2.2 are attached on which are marked (with an arrow in each case) locations of one or of the following conditions: damaged wall surfaces, incomplete paint application, patched surfaces awaiting painting, scrapes and the presence of foreign materials. In some cases these conditions occur behind baseboard radiation. Exact locations are not provided due to the volume of such conditions. It is the Contractor's responsibility to complete all Work required under the Contract. Room 100 Vestibule • Provide Floor Mat Unit. Room 101 Rotunda • • Provide continuous vinyl base to each face of the Millwork Counters that face the Rotunda. Room 105 Men • Repair scratched paper towel dispenser face. Room 106 Women • Replace bent sanitary napkin disposal unit. Room 119A Entry • Provide Floor Mat Unit. Room 141 Finance • Provide missing door in wall cabinet located on north wall. Room 145 City Council • Millwork Dais unit is not placed correctly upon its base: Modify that base so that each projection of the Dais above the base is uniform. • Remove debris and dirty prior to painting at back of curved wall in southeast corner of room and also at southeast wall. • Provide handrails. • Provide continuous metal trim at top of fascia walls: tops are ragged and have holes and gaps. • Patch holds around structural elements they penetrate fascia walls. • Paint fascias the same cream color as on interior walls of this room (instead of currently- applied white color). • Paint northwest fascia cream color to cover currently- applied black color. • Provide wood filler at nail holes in wood trim. • Repair damaged surface below Dais millwork unit. Openings • Provide frames, doors and finish hardware where not currently installed at each applicable Opening. • Provide incomplete hardware at each respective Opening. • Tighten up each lever handle: many are loose. • Remove foreign materials, debris, stains and dirt from each threshold. • Provide transmitters for each Overhead Door. • Provide rubber button bumpers and gaskets at each applicable location. • Wood door faces and edges are rough to touch (sand embedded in finish ( ?): smooth down without ruing finish. • Repair and repaint scratched and chipped surfaces at bottoms of hollow metal door frames at each Opening: All openings are affected. Repaint each entire frame. • Repair and repaint scratched and chipped surfaces at various locations on hollow metal frames at each Opening especially at glass stops: All openings are affected. Repaint each entire frame. • Provide missing screws at glass stops and paint at Openings: 4/ Mechanical Mechanical Engineering Consultant's Final Inspection List • Attached is the "Final Mechanical /Electrical Inspection Report for Oak Park Heights (dated May 7, 2010). General Items • Provide floor escutcheon cover at each instance of hot water heating piping that penetrates floor finish materials (especially at Room 145 Council Chamber). • Complete installation of baseboard radiation units. Electrical Electrical Engineering Consultant's Final Inspection List • Attached is the "Final Mechanical /Electrical Inspection Report for Oak Park Heights (dated May 7, 2010). Hierseman/Dan Moulton M & E COMMISSION NUMBER: 12908 INTRODUCTION The purpose of this visit was to observe the Mechanical and Electrical installation for the Oak Park Heights City Hall. This report shall in no way relieve the Contractor of responsibility for completion of any and all work according to the terms of the Contract. The City Hall building is 98% complete or more. This list does not include obvious items to finish like controls, insulation, etc. • This list does not include the Public Works Building. PUNCH LIST GENERAL ITEMS Date Completed 1. Balance air and water flows and submit report. 2. Operation and maintenance manuals. 3. Owner instructions. See Specification. 4. Extra filters for all air handling equipment. See Specification Section 15801, 3.01. 5. Clean dust from all equipment and controls. 6. Field testing and verification documentation of electrical equipment and systems is required, but will not be done until later. Submit testing documentation. • 1 of 3 • MECHANICAL ITEMS 1. Provide final connection of water to humidifier in duct. 2. Provide relief valve for boiler and pipe to floor. 3. Provide corner piece of radiation in room 142. 4. Provide radiation in room Break Room 126. 5. Miscellaneous pieces of radiation and some trim pieces are missing. Install when available. 6. Some of the radiation has chips in the paint and will need to be touched up. 7. Install radiation in Council Chambers. 8. Verify all plumbing fixtures are operating with sensors and have correct flow. Set hot water temperatures to required settings. 9. The following is the temporary conditions issued on September 17, 2010 to allow the system to operate with reduced cooling capacity until the additional wells are completed after the existing building is removed. • Temporary air flow to the lower level while second phase of the geothermal well system is being completed. This will provide exhaust for the locker room and evidence rooms and provide air flow in the corridors to provide a positive pressure against the garage. 1. ERV -1 to operate as designed. 2. Provide a 20 x 12 bypass duct from the 22 x 14 exhaust to the 18 x 12 supply above room 019. 3. VAV box 001 to be full open. 4. Cut a 10 x 10 opening in the 46 x 14 duct above the ceiling in Hall, 024 and Women's Locker room, 016. Provide a 24 x 24 eggcrate in the ceiling of the Hall, 024 and the Women's Locker room, 016. 5. Connect a 20 x 10 bypass duct from 22 x 14 duct in 019 to 16" round duct in 019 (from VAV 002). Close and shut down VAV 002. 6. Exhaust fan EF -10 will be off. 7. When the geothermal system is complete, remove the bypass ducts, the (2) eggcrates and return the VAV box controls to normal. Patch ductwork as required. Operate EF -10 as designed. 8. To reduce the extra cost of balancing the two VAV boxes and EF -10 will not be balanced until after the system is back to normal. ELECTRICAL ITEMS 1. Clean light fixtures. There is debris inside the direct/indirect light fixtures and the glass in the council chambers light fixtures is dirty. 2. Most of the systems are incomplete and in progress. A follow -up site observation will be required to verify completion. • 2of3 • 3. The voice /data outlets are both blue. Provide a for telephone and Y P white for data. Refer to Specification Section 17741, 2.O5,G. 4. Installed receptacles are white. Specified is gray. 5. Specifications do not specify the quantity of amplifiers, preamps, etc for the public address and music systems. The quantity required is as necessary to operate the system. The door bell for the Police Reception is to sound in rooms 102, 116, 110, and 122. The door bell for the City Hall Reception is to sound in rooms 125, 126, 130, 133, 134, and 141. If the Owner signs off on different locations for sounding of these door bells, at least provide separate wiring for future operation as indicated.
