AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed
according to: (1)the conditions of this permit;(2)the
approval plans and specifications;(3)the applicable city
approvals,Ordinances,and Codes;and,(4)the State Building
Code. This permit is for only the work described,and does
not grant permission for additional or related work which
requires separate permits.This permit will expire and become
null and void if work is not started within 180 days,or if work
is suspended or abandoned for a period of 180 days any time
after work has commenced. The applicant is responsible for
assuring all required inspections are requested in conformance
with the Minnesota State Building Code.

Re: Building Permit Application- Conference Room Wall- Description of work
Oak Park Heights P.W. staff will remove the non-load bearing wall separating conference
rooms 144B and 143B. All electrical work shall be completed by a licensed electrician and
inspected in accordance with all applicable MN statutes,building codes, City of OPH codes,
rules and regulations and or protocol. The drywall seams will be patched to smoothly
transition existing walls and soffits, creating one conference room. Finish work will include
painting,patching carpet,reinstalling TV's,white boards and A/V equipment.
The conference room wall removal project consists of the following:
• Drywall removal
• Remove/replace electrical and A/V wire and cables(Sub-Contracted)
• Remove studs that separate conference rooms
• Existing drywall seam patching
• Finish work ie. paint, carpet patch, reinstallation of appurtenances (Some may be sub-
contracted)
