Dear Ms. JoJo Thuong Vo; This letter is a follow-up to the ownership change inspection conducted on February 26, 2015 at Sky Dragon (Mr. Chow Chinese to Go), Oak Park Heights, MN. The following items require compliance: 1. Provide casters or 6 inch stainless steel legs for front counter beverage cooler. If replacing, then provide equipment specification sheet detailing NSF certification. Comply with by opening. 2. Clean the following to be free of residues and splashes; Comply by opening. a. Floors in entire kitchen area. b. Walls around mop sink and back splash at cook line. c. Wall behind cooking equipment. d. Ceiling area in front of ventilation hood. e. All parts of microwave table. f. Interior of cook line ventilation hood and hood filters. g. Interiors of all reach-in refrigeration including shelving. h. Clean all parts of restroom including sink, toilet, fixtures, floors, and walls. 3. Provide wall mount hand soap dispenser for cooking area hand sink. Comply by opening. 4. Replace missing light bulb in Superior reach-in freezer. Comply by opening. 5. Repair broken adjustment knob on cook line reach-in cooler. Comply by opening. 6. Submit the Washington County Public Health and Environment food establishment license application, plan review application, and copy of menu for review and approval. 9. Ensure all necessary permits and approvals are acquired from the City of Oak Park Heights Building Official. 10. Existing license is not transferable; therefore a fee of$581.00 must be submitted.
